Decreased Throidal Response to Thyrotropine in Type ¥± Diabetes Mellitus

Abstract
It was recently observed that uncontrolled diabetic patients were chracterized by multiple abnormalities of thyroid hormone regulation, thereby, reported that baseline T_(3), T_(4), TSH, TBG were similar in both diabetes and controls. Also, it is disclosed that thyroid response to TRH was slightly decreased in diabetes compared with healthy controls.
Our aim was to verify as to whether there were these abnormalities of thyroid hormone regulation in the Diabetes, by means of observing thyroidal response to TRH stimulation.
We studied 83 patients with Type ¥± Diabetes Mellitus, ranging between 35 and 78 years-old (mean 48.9) and 20 healthy controls, respectively.
The mean values of baseline serum to T_(3), T_(4), TSH, TBG in the diabetics were 1.15¡¾ 0.43 ng/ml, 9.65¡¾0.76 ug/ml, 3.39¡¾0.53 uIU/ml, 22.97¡¾2.45 ug/ml and maximal serum TSH mean value after TRH infusion was 12.87 ¡¾3.37 uIU/ml, and mean TSH max/TSH base ratio was about 3.8. The mean values of those in the control group were 0.96¡¾0.65 ng/ml, 9.09¡¾0.94 ug/ml,3.74¡¾0.54uIU/ml, 21.9¡¾3.45 ug/ml and maximal serum TSH was 22.01¡¾4.45 uIU/mI, and mean ratio was about 5.9.
There were no significant differences of baseline serum T_(3), T_(4), TSH, TBG levels between the patients with type ¥± diabetes mellitus and the control group (p <0.01). However, serum TSH response to TRH stimulation in patients with diabetes mellitus was slightly decreased than the control group (3.8 vs 5.9, p< 0.03).
In our study, thyroidal response in type ¥± diabetes meltitus was slightly decreased than normal subjects.
There results also suggested that the caution must be used to check the thyroid function and be needed to serum glucose carefully in the diabetes patients.
